The candidates fulfilling qualifications shall have to bring all necessary certificate & documentary proof (original + two attested copies) i.e. (1) Present pay scale and pay slip of last salary drawn. (2) School Leaving Certificate. (Or any other proof of birth date) (3) Caste certificate. (4) Degree certificates and mark sheets of MBBS and PG degree. (5) Attempt Certificate, and Registration Certificates of MBBS and PG degree.(5) Experience certificate. (6) Research Publications if any. (7) NOC from present employer for appearing in this interview (8) Applicant presently not serving elsewhere and had previously served shall submit relieving order of previous institution. (8) Photo I.D. as prescribed by M.C.I. (9) Proof of change in name/Proof of change in marital status at the time of interview. (10) If applicant appointed in SMIMER, he/she should invariably produce the relieving order/letter from present employer. A candidate who knowingly or willfully furnishes incorrect or false particulars or suppresses marital information will be disqualified and if appointed he/she will be liable to dismissed from service without any notice.
